Medicare Part B note: Patients with diabetes or chronic kidney disease are eligible to receive nutrition counseling/medical nutrition therapy under Medicare Part B, as long as a physician referral is in place prior to your first visit. If you have a Medicare Advantage plan, but do not have one of these diagnoses, please call us and let us help you determine whether your Medicare Advantage plan may cover medical nutrition therapy for your diagnoses.

Responsive Patient Scheduling

Clinical notes describing your patient's progress will be faxed to you on a regular basis to ensure continuity of care.
